1. In the course of volunteering, I may be required to handle personal data (including sensitive personal data) and official information that is governed by the Official Secrets Act (in particular, section 5 of the Official Secrets Act).
2. I understand and agree to abide by the terms stated in paragraphs 1 to 5 below:
1. All official information and personal information acquired by me in the course of House Visit is of a strictly confidential nature and is not to be disclosed or communicated by me to any person in any form whatsoever.
2. All personal data and official information can ONLY be used for the sole purpose of assisting the residents.
3. All personal data and official information must be treated with utmost care to prevent loss or unauthorised access, use, modification and/or disclosure.
4. All personal data and official information must be deleted, destroyed or returned to the People’s Association as may be required by them.
5. I will comply with all relevant provisions in the Personal Data Protection Act 2012 and Official Secrets Act.

I further understand and agree that any breach or neglect of this undertaking may render me liable to prosecution under the Official Secrets Act and or the Personal Data Protection Act 2012 (as may be applicable).
